We are looking for an organised, self-motivated and confident individual to join our Social Prescribing team in a specialist mental health role. You will work with adult patients to help them access support that will enable them lead more independent, resilient and healthier lives.

Working alongside GP practices, Social Prescribing Link Coordinators help patients and families connect with health and community services which will make a real difference to their emotional and physical wellbeing.

You will deliver personalised support to patients, independently responding to new referrals from health professionals, and managing a caseload. It’s essential that you have an understanding of how to prioritise need, are able to recognise when to refer on to specialist services, and must be confident to communicate at all levels across health and social care sectors. 

The position is available in Gillingham; applicants must have a background in medical or social care, and experience of delivering mental health support to adult clients or groups.

You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
