Job Title: Specialist Healthy Lifestyle Practitioner – T3 Child Weight Management
Location: Medway & Swale – Exact location to be decided following interviews
Hours: Permanent - Full time/Part time
Reporting To: T3 Clinical service lead
Key Relationships: Strategic Children’s Service Manager, T3 clinical Service lead, Children's Healthy Lifestyle Manager, T3 Senior healthy lifestyle practitioners, tier 3 service dietitian, tier 3 service family therapist, tier 3 triage nurse.
The Role:
The Tier 3 Specialist child weight management service offers a comprehensive multi-disciplinary team approach combining a specialist dietitian, family therapist, specialist nurses, and healthy lifestyle practitioners. The service goal is to support children and their families to achieve moderate and sustainable weight loss using a psychologically informed approach. Support is provided within a group and individual setting over a 12-month period. The service supports children and families with complex social, medical, and educational needs. The post holder will work alongside the second healthy lifestyle practitioner and specialist nurse to coordinate and deliver the service in their area.
• To co-ordinate and deliver tier 3 weight management programmes to children, young people and families in Medway & Swale.
• To work as part of the tier 3 team to support children, young people and families to make healthy lifestyle changes that support a heathy weight through the delivery of an evidence based psychologically informed curriculum.
• To coordinate the set up and delivery of programmes within your locality based on service needs.
• To work as part of the tier 3 team to monitor the progress of children and young people through collection of data.
Key responsibilities: -
Responsible for coordinating and delivering group and one to one T3 child weight management programmes within locality. Covering other localities as required.
Develop a plan of the programmes and venues required to meet demand and allocate delivery staff on the programmes accordingly.
Monitor referral numbers to ensure programmes meet necessary demand.
Organise the resources, equipment and venues required to deliver the programmes and ensure the costs are managed within the budget allocated.
Collect, collate, and upload data from service users, including weekly anthropometric measurements and questionnaires to monitor changes in healthy lifestyle behaviours at baseline and at 3,6 and 12 months.
Undertake administrative tasks, such as service update letters, maintain effective recording systems and respond to queries from clients and professionals.
As required provide input into service improvement and curriculum development.
Develop a relationship with young people based on respect and trust.
Establish boundaries and challenge inappropriate behaviour.
Mentor, coach and support individuals, encouraging healthy lifestyle change and addressing barriers to change. Maintain regular contact with clients.
