Adults - Social Care Officer

Exciting opportunity to join the expanding Transitions Team within Adult Social Care. You will be responsible for reviewing of those Young people (18 – 25 year) who were previously known to the Disabled Children's team. 3-month contract, extension possible.

The Social Care Officer is responsible to the Social Workers (s) for the day-to-day performance of his/her duties to service users within the community.

You will work collaboratively with multidisciplinary teams both in the community and or within the hospital setting in facilitating timely and effective care and support plans to maximise independence at home or to return home with ‘own bed is best' ethos including the development and monitoring of re-enablement programmes.

It's your responsibility to ensure vulnerable people are safeguarded and promote safeguarding personal values an ethics through day-to-day work.

Main purpose of the role - To assess the care and support needs of adults, including those with mental health needs, people with physical and learning disabilities, and their carer's needs within the community. As part of a professional team, you will ensure client and carers' wishes are central to the decision-making and together formulate and implement appropriate care plans which will maximise their independence and help them to remain living in the community for as long as possible, or assist them to move seamlessly into more appropriate accommodation.

Qualifications
Essential
• NVQ Level 3 in Care or equivalent Level 3 qualification
• Trusted Assessor training (or equivalent experience with equipment and minor works) or willingness to work towards.
• Must be able to travel throughout Medway.

Experience
Essential
• Experience of delivering direct work to vulnerable adults
• Experience of contributing to assessments and enabling support Demonstrable experience or understanding of the field of care management
• Ability to demonstrate the practical and/or procedural knowledge across a technical or specialist area and has the ability to undertake a wide range of activities, or an equivalent level of organisational, procedural and policy knowledge.

Skills
Essential
• Demonstrable ability to use judgemental, analytical or creative and developmental skills to interpret information or situations and to solve varied problems or develop solutions or plans over the short term.
• Demonstrable ability to work within recognised procedures which leave some room for initiative, including lone working, previous experience of responding independently to unexpected problems and situations and only referring to a supervisor/manager for unusual or difficult problems.
• Excellent customer care skills, with evidence of creative thinking and experience of adapting services, where possible, to meet customer needs and can take the initiative to work with other agencies where necessary.
