We are recruiting a Social Prescribing Care Coordinator to work with patients living with cancer and receiving cancer treatment. It’s an important role in patient coordination and care. You will join an established Social Prescribing team, and work in surgeries alongside the Medway South Primary Care Network (PCN) and practice teams.

You’ll be in daily contact with patients experiencing a range of cancers, talking to them about their condition, developing care plans and helping them understand pathways of care; ensuring patients are consulted, empowered and heard. Through the provision of personalised care, patient voice and shared decision-making, you will help patients participate more fully in their own health and wellbeing, and actively engage in treatment.

You will work with closely with primary care services and hospital team members, coordinating interventions, updating records, and evaluating interventions to provide the seamless integration of services around cancer patients. You will also support and signpost patients by phone, progressing their care pathway in liaison with other colleagues in the PCN and departments in the hospital.

There will be opportunities to carry out audits to review and promote best practice, and to liaise with organisations such as Cancer Research UK.

Applicants must have experience of working with people living with cancer and cancer care; and be able to support people in a way that inspires trust and confidence. You should have excellent communication skills, be compassionate, motivated and able to work independently and as part of a team. Car driver essential.
