We are looking for maternity cover in our Community Care Navigation teams, working with adults with complex needs and adult carers.

The roles are largely community based, with some office-based or home working. You will carry out a mix of home visits and assessments, communicating with clients and families virtually or by phone, signposting and making referrals, and liaising with other professionals and services

Applicants must be able to establish relationships quickly, communicate effectively and build trust. You should be proactive, motivated and able to work well as part of a team and on your own.

You will need to demonstrate a good understanding of confidentiality, data protection, safeguarding and boundaries in all aspects of your work.

These are part time roles with fixed term contracts for 9 months, with a potential 3 month extension. Salary is pro rata.

Positions are available in north Kent and Medway, and post holders are expected to be flexible regarding location. Driver with access to their own car is essential.
