This is a new and exciting opportunity in children’s mental health provision. As a Social Prescriber, you will support children and young people to develop resilience and improve their mental wellbeing, and through early intervention help prevent crisis situations.
You will work alongside multiple GP practices, independently managing a caseload and new referrals, providing personalised support and connecting children and families with health and community services to improve children’s mental health. The service helps address medical, social, environmental and individual needs, promoting good mental wellbeing and management of mental health problems.
We are looking for organised, self-motivated and confident individuals to join our established team. The positions is based in Sevenoaks, and applicants must have experience of working with children, a background in medical or social care and experience of delivering mental health support to clients or groups. It is essential that you have an understanding of how to prioritise need, and be able to recognise when to refer on to specialist services, being confident in your ability to communicate at different levels across both the health and social care sectors.
You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
