When someone’s mental health declines, it could be due to the effect of ongoing social problems - such as housing issues, debt or becoming isolated from others. The role will provide support in these areas, as well as mental health needs, alleviating the pressures people face, promoting recovery and wellbeing. As part of our mental health link team, you will support people with issues related to mental health, physical health, finance, housing, employment, as well as referring and signposting to other services in the community.
You’ll work with people on a one-to-one basis, assessing their needs, setting goals and helping them become more involved with the local community. This role involves working closely with other professionals and organisations, including primary care networks, to make sure people are receiving the most appropriate support for their needs.
The role requires:
- Good communication and people skills
- The ability to develop constructive relationships with the people we support
- The ability to develop strong working relationships with other organisations
- Good literacy, numeracy and IT skills.
Interview date: 27th April 2021
Contract: Permanent
Working Pattern: Monday to Friday 9am to 5pm
