Support worker for a Hospital Discharge programme, supporting individuals across the East Kent area.

Support Worker - Adult Hospital Discharge, Physical Disabilities and Mobility Issues
East Kent Area
Temporary ongoing role
£9.77 per hour (inclusive of holiday pay)

Organisation/Company:
One of the country's largest charities are seeking support workers across the wider East Kent area to assist with their upcoming project. The charity currently provides a service across the area caring for individuals who have recently been discharged from hospital who need assistance in rehabilitating back into their everyday lives.

Role:
Working across the community you will be supporting adults who have recently been discharged from hospital and assisting them to re-enable independent living following their hospital stay. You will offer practical and emotional support to individuals and may be required to transport individuals too and from their home (a vehicle will be provided for this). Your daily duties will vary depending on the needs of the individual but health and safety and spotting risks within the home and reporting this correctly will be a key part of your role. You will be working with individuals across East Kent including Thanet, Margate, Broadstairs, Ramsgate, Herne Bay and Canterbury it is essential that you have your own vehicle as you will be required to work from three different sites/bases and will need to travel between these.

Work Experience / Skills Required:

* A minimum of 6 months experience within a similar care setting in the last 2 years (Hays must gain references to cover this experience).
* Knowledge of health and social care, e. g. how to transfer the patient safely, how to help the patient to sit on a chair
* Experience in working with vulnerable adults, people with mobility issues and people with physical disabilities
* Awareness of moving and handling
* NVQ Level 2 or Level 3 are desirable but not vital.
* The sensitivity required to provide a trusting and stimulating environment along with being passionate about supporting people.
* Excellent communication skills and the ability to be a good team player
* A genuine desire to make a real difference in people's lives
* You will be positive and willing to create and follow a personalised service to every individual that you support.
* Be available to support in whatever capacity is required by an individual to nurture their path to independence.

* Monday - Friday work schedule 35 hours per week
* You must be flexible to work hours between 11am-7pm
* Work vehicle supplied for work use
* The position is an ongoing temporary role.
* DBS support if you are not on the update service.
* Access to online training courses
