We have 2 exciting opportunities to join our Social Prescribing team working with people with complex needs, helping them to access the support they need to live independent and healthier lives.
Positions available in Rochester and Ashford.
You will work alongside multiple GP practices, independently managing a caseload and new referrals, providing personalised support and connecting patients with community services to improve independent living, health and wellbeing. It is essential that you have an understanding of how to prioritise need, and be able to recognise when to refer on to specialist services.
You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work.
