We are looking to recruit a Care Planning and Assessment Leadto cover 12 Months Maternity leave.

All Seasons is an established and respected local community care organisation based in East Kent. We work closely alongside colleagues in the NHS, social services and have a number of innovative projects planned to develop our services and technology with health.

We need someone who is experienced in social care to join our care planning team to cover maternity leave. This is a dedicated team who carry out client visits to put in place new care plans or replace existing care plans, carry out client reviews. The ideal candidate will need a keen eye for detail, strong verbal and communication skills and a solid background in health and social care preferably with experience at working to supervisory level.

This role will be dedicated to providing care planning and risk assessment support as well as providing quality assurance spot checks. You will not be delivering care, but will form part of our dedicated QA team led by our Head of Care. You will also be able to work alongside other health and social care professionals and arrange assessments and case reviews where needed, liaising with the Senior Management Team where required to ensure that all records are kept up to date at all times.


Due to the nature of the role the working hours will be from 9am to 5pm Monday to Friday. Although there may be the occasional need to support with assessments and care plans in the evening as part of unexpected or delayed hospital discharges.Coaching and training will be provided to the right person.


The salary is £19,570 per annum and we offer a stake holder pension scheme, childcare vouchers, free training. We provide a company car for the sole use of extensive travel so you must hold a full diving licence. You will also be provided with a company mobile phone, lap top and mobile printer.
