This is a great opportunity to join the experienced team at Imago, a leading charity providing support and opportunities to people across the South East.
Specialising in Young Adult Carers referrals, you will support both carers and older people and adults with complex needs.

You will meet with individuals in their own homes to carry out holistic assessments and statutory Carers Assessments, working with them to identify what is going to make the greatest difference in their lives to improve health and wellbeing; building trusted relationships to improve confidence and motivation, encourage personal choices and access a range of community, health and social care options.

Working within a local team, you will manage a caseload and new referrals; implementing action plans, offering information and guidance, and emotional and practical support, enabling people to have greater choice and control to attain personal goals.

Working with an understanding of issues affecting Young Adult Carers, you will respond to challenging and complex situations with a sensitive and measured response, in line with Imago Safeguarding and Lone Working Policies.

Experience of working with young adults, knowledge of local services and the ability to build effective networks with cross sector organisations is essential.
