Part Time Vacancy Available
This is an exciting opportunity to join an experienced team working with older people and adults with complex needs. Imago is a leading charity providing support and opportunities to people across the South East. You will be responsible for a dedicated area, and following training, will independently manage a caseload and new referrals.
Referrals are received from GP’s and other health social care professionals. You will meet with clients in their own homes to carry out holistic assessments and together design plans with the aim of improving health and wellbeing. You will build trusted relationships to improve confidence and motivation, encourage personal choices and to access a range of community, health and social care options.
You will respond to challenging and complex situations with a sensitive and measured response, in line with Imago Safeguarding and Lone Working Policies.
Knowledge of local services and the ability to build effective networks with cross sector organisations is essential.
