Can you help others to make positive changes?

Social Prescribers connect people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing. You will be working with GP surgeries in the Sevenoaks Primary Care Network, and specialising in supporting adult mental health patients. Applicants need to have a good understanding of mental health conditions and practical and psychological barriers.

Using a holistic and individualised approach, you will help patients identify what changes they want to make, then support them to take action. You’ll connect them with services that meet their needs, such as financial and benefit advice, housing, health and safety home systems, and leisure and social activities. In the process, patients are supported to develop confidence and resilience, and improve their health and wellbeing outcomes.

You will respond to new referrals from health professionals and manage a caseload. It’s essential that you understand how to prioritise need, can recognise when to refer on to specialist services, and are confident communicating at all levels across health and social care sectors. 
Applicants must have a background in medical or social care, and experience of delivering mental health support to adult clients or groups. A car driver is essential.
