Are you the person we’re looking for?

We are recruiting for someone with excellent communication and organisational skills to join our Social Prescribing team. As a Care Coordinator, you will work in GP practices, coordinating support and services, ensuring patients do not fall into any gaps while waiting for ongoing support.

A key aim of Care Coordination is to help patients engage more fully in their own health and wellbeing, so you will provide extra time to help patients understand the conversations they have with primary care professionals.

You’ll provide information and navigation support to patients, helping them make choices and decisions about their care and support needs, creating personalised support plans together, and supporting them to access other community or social activities that will aid recovery.
You will work alongside Social Prescribing colleagues and other professionals, and it is essential that you are able to communicate at all levels across both the health and social care sectors.

Applicants must have experience of working with vulnerable adults and a background in medical or social care, and experience of care planning would be an advantage. The role is based in Rainham (Kent) and a car driver is essential.
