Porchlight has an exciting opportunity to expand the successful Live Well Kent service into Medway on behalf of Kent & Medway CCG, leading to Live Well Kent & Medway - a network of services (led by strategic partners Porchlight and Shaw Trust) that helps people who are struggling with their mental health and wellbeing.


The role: You will manage and develop a small team of mental health link workers who support people with their mental health, including issues that may be affecting this, such as finance, housing, employment, and loneliness.

You will oversee referrals into the service; some will be people seeking support for themselves; others will be made by GPs, as well as other professionals and voluntary sector organisations. You will review and manage referrals to the team, as well as to our delivery network.

You will also review and report on referral trends and needs. This will help us to improve the support that is available to people in Medway.
This role involves working closely with other professionals and organisations in the network, including local primary care networks, to make sure people are receiving the most appropriate support for their needs.

This role requires:

- Good communication and people skills
-The ability to develop strong working relationships with other organisations
- Knowledge of health & social care organisations in the area that support people who have mental health needs
- Strong organisational skills (including the ability to use your own initiative and prioritise your own workload)
