We are looking for an organised, self-motivated and confident individual to join our established Social Prescribing team working with adults with complex needs, helping them to access the support they need to live independent and healthier lives.  

The position is available in Ashford, and applicants must have experience of working with adults, and a background in medical or social care.

You will work alongside multiple GP practices, independently managing a caseload and new referrals, providing personalised support and connecting patients with health and community services to improve independent living, health and wellbeing. It is essential that you have an understanding of how to prioritise need, and be able to recognise when to refer on to specialist services, being confident in your ability to communicate at different levels across both the health and social care sectors. 

You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
